We are looking for passionate nurses to help us expand the reach of independent physicians embarking on chronic care management programs with CCI.
This is an independent contractor 1099 position.
Overview
RN to provide clinical services in compliance with the CMS Chronic Care Management Program (CCM) with sincere compassion and a passion for helping seniors maintain optimal health.
Expectations
Patient Outreach: Telephonic outreach to patients with 2 or more chronic conditions including CCM patient enrollment, care plan development, and EMR documentation
Conduct monthly non-face-to-face chronic care coordination patient calls including:
Review and update Care Plan
Goal setting and follow up in accordance with patients’ abilities and willingness to comply.
Coordinate care: transitions, appointments, and community resources.
Patient education on chronic conditions
Communication in the Medical Practice EHR for continuity of care issues that arise and require Medical Practice follow-up
A time commitment on a mutually agreed-upon, consistent number of hours per week.
Requirements:
RN Licensure (unrestricted) required - Compact License preferred
Minimum 20 hours commitment per week
Maintain Medicare compliance: HIPAA, Privacy, Security
Must have demonstrable clinical expertise working with Medicare eligible patients.
Must have own computer and consistent access to high speed internet
Home Office working environment - maintaining HIPAA and Medicare Compliance
Experience working with physician practice EMRs preferred but not required
